Job summary

The PACE Center CNA provides direct personal care and supportive services to participants at the PACE Center, under RN supervision. It promotes safety, independence, and engagement in center activities while documenting accurately for regulatory compliance.

Responsibilities include assisting with daily living activities, observing changes, and ensuring a clean, safe environment. Transportation assistance may be required as needed, with scheduling flexibility.

Qualifications

• High school diploma or GED required.
• Must be at least 18 years old.
• SRNA certification through Kentucky Board of Nursing required.
• Completion of an approved CNA training program required.
• Minimum of one year of experience with frail/elderly or training upon hire.
• Food handling permit required (upon hire).
• CPR certification maintained.
• Ability to work independently with minimal supervision.
• Strong communication with adults/elderly.
• Ability to follow written/verbal instructions and safety procedures.
• Ability to document accurately and timely.
• Collaborate with PACE staff and IDT.
• Strong interpersonal and communication skills.
• Ability to handle schedule changes and emergencies.

Responsibilities

• Provide direct personal care and supportive services to participants at the PACE Center.
• Assist with activities of daily living and promote safety and independence.
• Observe and report changes in participant conditions.
• Maintain a clean and safe environment and document in accordance with regulations.
• Assist with participant transportation as needed.
