Job summary

Midland Care Connection, Inc. in Emporia, KS is seeking a Certified Nursing Assistant for the PACE program. Responsibilities include assisting participants with activities of daily living and monitoring health changes, in a day shift setting.

We value compassionate care, adherence to individualized care plans, and teamwork with the interdisciplinary team to support older adults at home and in the center.

Qualifications

• High school diploma or GED required.
• CNA or HHA certification, or ability to obtain within onboarding period.
• At least one year of direct care experience with older adults, individuals with chronic illness, or persons with functional limitations.
• Ability to assist PACE participants with activities of daily living (ADLs).
• Demonstrated understanding of safety practices and health screenings in healthcare settings.

Responsibilities

• Assist participants with daily activities such as bathing, dressing, grooming, and mobility.
• Monitor vital signs and report health changes to nursing staff.
• Provide meal assistance and accompany participants during activities.
• Support to ensure comfort, safety, and independence in both center and home environments.
• Follow individualized care plans and directions from nursing, therapy, and IDT members.
