Job summary

The role includes documenting services, supporting transportation, behavioral goals, and mentoring activities, while promoting community engagement and person-centered planning.

Qualifications

• High School Diploma, GED or equivalent.
• Valid Driver’s License and acceptable driving record.
• Reliability, willingness to learn, and being open to new opportunities.

Responsibilities

• Complete required documentation and support the delivery of individual and group-based services, transportation, behavioral support, skill development, mentoring, and recreational activities.
• Implement activities that promote community engagement, relationship building, personal growth, and the development of valued social roles.
• Assist in the development and implementation of individualized service, support, and goal plans.
• Participate in the program's on-call rotation.
