The role supports individuals with complex medical, behavioral, and mental health needs, delivering person-centered care in home and community settings.

Qualifications

• High school diploma required; degree in a relevant field preferred.
• 1–2 years of experience with people with complex needs; 3–5 years preferred.
• Must pass background check (BCA) and hold a valid driver's license with MVR check.

Responsibilities

• Assist individuals with daily living skills such as meal prep, personal hygiene, and community navigation.
• Advocate for individuals' rights and medical needs.
• Complete shift documentation and record-keeping.
• Manage crisis safely; training provided.
• Engage in trauma-informed care daily.
