Job summary

Northgate Health Care Facility is seeking an LPN Unit Coordinator for the night shift. You will oversee the daily clinical operations of a nursing unit and lead nursing staff to deliver resident-centered care.

This role includes coordinating with physicians, families, and interdisciplinary teams to maintain quality outcomes. Responsibilities include supervising staff, reviewing assessments, updating care plans, and participating in care plan meetings.

Qualifications

• LPN license required or very recent licensure.
• Leadership or supervisory experience preferred.
• Experience in clinical care and coordination is beneficial.

Responsibilities

• Oversee clinical care and daily operations of an assigned nursing unit.
• Supervise nursing staff and unit support staff with leadership and guidance.
• Monitor resident conditions and update care plans as needed.
• Coordinate communication with physicians, residents, families and other professionals.
• Review labs, orders, incidents, and changes in resident condition.
• Participate in care plan meetings, QAPI activities and family meetings.
• Assist with admissions, discharges, transfers and outside appointments.
