Qualifications

• MSW degree and NYS licensure (LMSW or LCSW) required.
• Spanish speaking preferred.
• Experience in ED social work and discharge planning is a plus.

Responsibilities

• Facilitate discharge of patients in the ED.
• Conduct psychosocial assessments and a full range of interventions for patients and families.
• Coordinate transitional care, follow-up calls, and post-discharge interventions.
• Provide counseling and crisis intervention, including domestic violence, child abuse, sexual assault, human trafficking, and death.
• Assist with access to services and placement in facilities; coordinate with outpatient social workers and Care Coordination.
