Job summary

Beth Israel Lahey Health is recruiting a licensed social worker to provide psychosocial screening, assessment and counseling for patients and families. You will collaborate with physicians, nurses, and case managers to support safe, timely patient care and discharge planning.

In this role you will join the overnight primary response team in the Emergency Department, triaging requests, conducting assessments, and contributing to CQI efforts.

Qualifications

• MSW degree and licensure as an LCSW or higher.
• Experience with hospital/ED social work and with computer systems (MS Office/SharePoint) preferred.
• Strong documentation and communication skills in electronic medical records.

Responsibilities

• Provide psychosocial screening, assessment, and counseling to patients and families.
• Collaborate with physicians, nurses, case managers, and community agencies for care planning and discharge.
• Meet productivity standards and document in the medical record.
• Participate in CQI efforts and staff education or teaching conferences.
• Supervise staff or interns when licensed as LICSW; mentor junior staff.
