Job summary

Mount Sinai Morningside seeks a social services professional to support patients and families, assist with discharge planning, coordinate community resources, and provide crisis intervention within a hospital setting.

The role emphasizes collaboration with care teams, documentation in the medical record, and education for patients about benefits and entitlements; prior health care or social service experience preferred.

Qualifications

• Related experience in the social service and/or health care field, preferably one year in a medical setting.
• Collective bargaining unit: SEIU 1199-MSSL

Responsibilities

• Help patients/families to meet concrete needs such as obtaining services, housing or employment
• May assist in obtaining medical equipment and support services
• Gather information about community resources and services and makes appropriate referrals with liaison and follow-up
• Assists with resumption of home care services
• Perform specific assignments relevant to particular programs
• Perform community based tasks to facilitate discharges
• Document in the medical record in accordance with standards set for the Department of Social Work and/or regulatory agencies
• Identify high risk situations such as child abuse, elder abuse, domestic violence and informs supervisor of these cases
• Provide patient/family education and encourages participation in care plan
• Maintain knowledge of social service policies related to benefits and entitlements and able to assist patients to secure needed services
