• Excellent clinical skills in rapid psychosocial assessment, crisis intervention, and focused short-term treatment.
• Ability to prioritize multiple urgent requests from a diverse patient population.
• Strong interdisciplinary/collaborative and organizational skills.
• Engaging patients/families; coordinating resources within hospital and community networks.
• Spanish speaking preferred.
• Internal candidates: minimum one year in current position unless in ED/RETU.

Responsibilities

• Responsibilities include psychosocial assessment; a full range of psychosocial interventions for patients/families; transitional care; and follow-up and post-discharge intervention.
• Services include counseling; crisis intervention; assistance with concrete services; placement of patients in facilities; close collaboration with outpatient social workers and Care Coordination programs.
