Job summary

Whole Life in Preston, CT is seeking a Direct Support Professional (ID 008-07) to assist adults with developmental and/or physical disabilities. This part-time position centers on ADLs, safety, community integration, and person-centered care within a supportive team.

Responsibilities include documenting care in English, supporting daily living activities, daily living activities, and assisting with medications under supervision.

Qualifications

• Proof of High School diploma or GED.
• Proof of valid driver's license, social security card, birth certificate, or passport.
• Ability to pass background checks and health screenings, including a Fit for Duty physical and TB test within 30 days of hire.
• Must be able to stand, move, twist, bend, kneel, reach, lift, and walk for the majority of the shift; occasional lifting up to 100 lbs.
• Must document care in English and communicate with clients and coworkers in English.

Responsibilities

• Assist with Activities of Daily Living (ADLs): bathing, dressing, grooming, toileting, and meal preparation.
• Administer medications, monitor vital signs, and support medical appointments.
• Ensure a clean, safe living environment through light housekeeping and laundry.
• Engage clients in social activities, community outings, and choice-making to promote independence.
• Document daily activities, observations, and incidents accurately.
• Maintain confidentiality per HIPAA regulations.
• Implement individualized care plans.
