Qualifications

• High School Diploma or GED preferred.
• Qualified New York State Driver’s license that is also qualified in accordance with agency’s insurance standards.
• Successful completion and maintenance of AMAP status.
• Ability to communicate and understand written and oral directives/information and an excellent team player with a positive attitude.

Responsibilities

• Provide ongoing support to people during overnight and morning hours, including self-care routines.
• Ensures a safe and clean home by assisting with household chores as needed.
• Follows all treatment plans and uses adaptive and safety equipment to promote independence and well-being.
• Reports and documents events/activities that may affect health or safety.
• Reports to the Justice Center as a mandated reporter if abuse or neglect is suspected.
• Adheres to Federal and New York State regulations, agency policy, and training requirements.
• Completes AMAP-related medication assistance according to policy.
• Arrives on time and adheres to call-off policies to ensure quality support.
• Supports morning meals, teaches diet adherence and portion control.
• Assists with medication administration per AMAP policies.
• Accompanies or transports people to day programs or job locations using safe driving.
• Documents treatment plans and other materials using Medisked or paper records.
• Participates in inter-disciplinary team meetings to discuss progress and goals.
• Mentors new Direct Support Staff and reviews training materials.
• Performs other duties as requested by supervisor.
