Job summary

Citizens Options Unlimited is seeking a Direct Support Professional to provide overnight and morning support for individuals in their home, assisting with toileting, meals, grooming, dressing, and safety to promote a high quality of life. You will follow treatment plans, document activities, help with household chores, and work as part of an inter-disciplinary team.

A NY driver’s license and AMAP certification are required, and you must pass background checks.

Qualifications

High School Diploma or GED preferred.
Valid New York State driver’s license.
Successful completion and maintenance of AMAP status.

Responsibilities

Provide ongoing support to people during overnight and morning hours, including self-care routines and safety.
Assist with meals, health, safety, and daily living activities to promote independence and quality of life.
Follow treatment plans and utilize adaptive equipment to support health and safety.
Report and document events/activities that may affect health and wellbeing.
Adhere to agency policies, procedures, and training requirements.
Arrive on time and manage call-offs/overtime to ensure consistent support.
Mentor and role model for new Direct Support Staff and review training materials.
Performs all other duties as requested by supervisor.
