Job summary

Citizens Options Unlimited in Commack, NY is seeking a Direct Support Professional to provide overnight and morning care and support activities to individuals with daily living needs.

You will assist with self-care, health and safety, meal preparation, transportation to day services, and documentation of activities. The role requires AMAP certification and a valid NY driver’s license.

Qualifications

• High School Diploma or GED preferred.
• Qualified New York State Driver’s license that is also qualified in accordance with agency’s insurance standards.
• Successful completion and maintenance of AMAP status.
• Ability to communicate and understand written and oral directives/information and an excellent team player with a positive attitude.

Responsibilities

• Provide overnight and morning support including self-care routines (toileting, feeding, showering, dressing, diapering).
• Maintain health, safety and well-being and ensure a high quality of life for individuals.
• Support with household chores and assist with meal preparation.
• Follow treatment plans and use adaptive equipment as needed.
• Document events affecting health and safety; report concerns.
• Adhere to agency policies and regulatory requirements; complete fingerprint/background check.
• Assist with medication administration according to AMAP policy.
• Transport individuals to day programs or job locations using safe driving practices.
