Job summary

U.S. VETS in Prescott, AZ is seeking an Outreach Specialist to connect veterans experiencing homelessness or at risk with housing and support services. This full-time role involves community outreach, data tracking, and coordinating transportation to program sites.

You will build relationships with veterans, shelters, and community partners, producing weekly reports and supporting special outreach initiatives. A 4-year degree and driver’s license are required.

Qualifications

  • Bachelor's degree in social services or related field.
  • Experience working with people experiencing homelessness and/or veterans.
  • At least one year in social services (employment at U.S. VETS for at least six months may substitute for one year of experience).
  • Strong oral, written, and communication skills.
  • Strong organizational skills and ability to work within a team.
  • Computer literacy and proficiency in Microsoft Office.
  • Valid driver’s license and ability to meet company insurance requirements.

Responsibilities

  • Publicize U.S. VETS services by visiting shelters and programs where homeless individuals may congregate.
  • Maintain a database of client information and referrals to track veterans and schedule screenings.
  • Coordinate transportation for veterans to U.S. VETS sites and events.
  • Collaborate with external service providers to collect paperwork for client files.
  • Produce weekly reports of veterans encountered by the outreach team.
  • Assist with communication of upcoming projects and events sponsored by outreach.
  • Attend weekly outreach meetings and participate in team activities.
  • Perform other duties as assigned.

What You’ll Do


  • Publicize U.S. VETS services by visiting shelters, treatment programs, transitional housing, sober living programs, soup kitchens, and other locations where people experiencing homelessness may congregate

  • Maintain a database of client information and referrals to track veterans and schedule screening appointments

  • Coordinate transportation for veterans to U.S. VETS sites and program events

  • Collaborate with external service providers to ensure timely collection of required paperwork for client files

  • Produce weekly reports of veterans encountered by the outreach team

  • Assist with communication of upcoming projects and special events sponsored by the outreach team

  • Attend weekly outreach meetings and participate in team activities

  • Perform other duties as assigned


Additional Program-Specific Duties

For Outreach Specialists working with SSG FOX clients:



  • Report to the head of clinical services or Director of Behavioral Health

  • Complete VA dashboard assessments, CSSR forms, PHQP, and intake psychosocial assessments

  • Build rapport and maintain relationships with clients and community partners

  • Experience working with people experiencing homelessness and/or the veteran population
