Job summary

USVETS Long Beach is seeking a Case Manager II to provide trauma-informed, strengths-based case management to veterans facing housing instability, trauma, mental health challenges, and other barriers.

You will conduct thorough client assessments, develop individualized service plans, coordinate services with the VA, landlords, employers and community resources, and perform home visits to support housing goals. Documentation and compliance are essential to success.

Qualifications

  • Bachelor’s degree in social work, psychology, counseling, or related field.
  • At least two years of work experience in case management or social services.
  • Ability to pass background checks for programs involving vulnerable populations.
  • Strong communication, documentation, and organizational skills.

Responsibilities

  • Provide trauma-informed, strengths-based case management to veterans.
  • Assess clients and develop individualized service plans (IAPs).
  • Coordinate services with VA, landlords, employers and providers.
  • Conduct one-on-one meetings and home visits to support housing goals.
  • Document services and incidents in MCR and other systems; maintain compliance.

Do you thrive in a collaborative, fast-paced environment where your clinical insight and case management expertise can make a meaningful impact?U.S. VETS the nation’s largest nonprofit dedicated to ending veteran homelessness—is hiring a Case Manager II to provide comprehensive support to veterans overcoming housing instability, trauma, mental health challenges, and other barriers to long-term stability.What You’ll DoAs a Case Manager II, you will provide trauma-informed, strengths-based case management to veterans Responsibilities include:Conduct thorough client assessments and develop individualized service plans (IAPs) addressing housing, employment, education, recovery, legal, financial, medical, and mental health needs.Coordinate and monitor services with the VA, landlords, employers, treatment providers, and community resources to help clients meet their goals.Provide regular one-on-one meetings with clients and conduct home visits (project-based and scattered-site housing).Conduct home visits at project-based units or scattered sites to ensure health and welfare of clientsTransport clients in company vehicles to appointments, interviews, court appearances and other meetings as needed.Facilitate life skills groups and/or therapeutic groups tailored to specialized veteran populations (e.g., female veterans, justice-involved veterans, families with children).Assist with crisis intervention, urgent client matters, and after-hours support as needed.Document all services and critical incident reports in a timely and professional fashion using MCR, and other electronic client record systems.Conduct random drug and alcohol screenings and ensure compliance with program rules and service plans.Support reporting, audits, and file reviews in collaboration with the Program Coordinator and Director of Behavioral Health.You AreMission-Driven – You are passionate about serving veterans and making a difference.Experienced – You have a solid foundation in case management, ideally with at-risk populations such as those experiencing homelessness, mental health disorders, or substance use.Organized & Adaptable – You can juggle multiple priorities, stay on top of documentation, and thrive in a team-based culture.Team-Oriented – You’re ready to support peers, collaborate across service team, and help elevate program quality.Required QualificationsBachelor’s degree in social work, psychology, counseling, or related fieldAt least two years of work experience in case management or other roles in the social services arenaAbility to pass background checks if assigned to programs involving incarcerated or recently released clientsStrong communication, documentation, and organizational skillsProficiency in Microsoft Office and electronic records systemsValid and current driver’s license with clean driving record to meet company insurance requirements; ability to complete and pass a driver training courseNot required but a definite PLUS!Experience working with homeless, veteran or justice-involved populationsCertification in Substance Abuse Counseling (e.g., CCAPP, CAADE)Experience facilitating groups or life skills classesWhy Join U.S. VETS?Comprehensive Benefits: Medical, dental, vision, 401(k) with match, generous PTO, and more.Grow Your Career: Gain valuable experience while receiving training and mentorshipSupportive Culture: Join a team of passionate professionals committed to collaboration, partnership, and the pursuit of excellence in our service to veterans and their families.Make a Daily Impact: Help at-risk veterans and their families find housing, healing, and hope.Join us to ensure no veteran is left behind.
