Job summary

Community Health Service, Inc. in Rochester, MN seeks a full-time MN Sure Navigator (Outreach and Enrollment Specialist) to connect residents with affordable health coverage.

You will enroll uninsured individuals, educate on coverage options, collaborate with local partners, and participate in trainings to advance outreach goals.

The role requires bilingual English/Spanish communication, reliable transportation, and strong computer skills; evening hours may be needed.

Qualifications

• HS diploma or GED required; equivalent combos considered.
• Bilingual English/Spanish communication and documentation.
• Reliable transportation and valid driver’s license.
• Strong computer skills with MS Office.
• Experience with social services is a plus.

Responsibilities

• Provide enrollment assistance for uninsured to access subsidized, low-cost and free health insurance programs.
• Offer structured patient education on health coverage and renewals for enrolled individuals.
• Distribute outreach materials to patients, clients, community members, partner organizations and businesses to raise coverage option awareness.
• Develop, discover and attend community events to promote coverage options and CHSI services.
• Collaborate with local organizations to build awareness of coverage options and referrals.
• Develop and assist in staff trainings and coordinate group meetings and support sessions.
• Attend training programs and participate in webinars for professional development.
• Accurately provide required reporting to track goal achievement and client satisfaction.
