Job summary

LUK, Inc. is seeking an Outreach Case Manager to engage and retain youth aged 16–25 in housing services. The role links clients to community resources, conducts screenings, and develops individualized service plans with ongoing progress evaluation.

The position requires strong communication, cultural responsiveness, and the ability to work both independently and in a team. The role is based in Central Massachusetts with office access in Fitchburg or Worcester and occasional remote work.

Qualifications

• Bachelor's Degree in Human Services or related field preferred.
• Case management experience with youth/young adults preferred.
• Bilingual/Bicultural candidates encouraged.

Responsibilities

• Provide case management and supportive services to youth and young adults in housing crisis.
• Engage and retain young people in services.
• Complete intakes and discharges promptly.
• Conduct assessments and develop individualized service plans.
• Coordinate with partners and connect to community resources.
• Participate in supervision, trainings, meetings and project activities.
• Assist with emergency services on-call rotation.
