Job summary

Community Teamwork is seeking a Housing Support Case Manager at the Youth Opportunity Center in downtown Lowell. The role focuses on case management for at-risk and homeless youth aged 14-25, providing advocacy, support, and connections to resources to help achieve stable housing and well-being.

Responsibilities include assessing housing needs, formulating individualized plans, and coordinating with partners to address barriers.

Qualifications

• High school diploma or equivalent required.
• 2+ years of experience working with at-risk youth.
• Ability to engage with at-risk youth in group and individual settings.
• Experience in assessment, advocacy and case management in Human Services.

Responsibilities

• Assess housing needs for each participant and develop plans.
• Create individualized housing stabilization service plans.
• Conduct regular youth housing visits in the community.
• Collaborate with Youth Services team and assist with complex cases as needed.
