Job summary

Stevens Treatment Programs is seeking a Case Manager for the Transitional Living Program in Fall River, MA to support adolescents and young adults as they prepare for independence, education, employment, housing, relationships, and community connections.

This active, relationship-based role combines individualized case management with hands-on work in the community, coordinating services, completing assessments, and contributing to treatment, transition, and aftercare planning.

Qualifications

• High school diploma required; Bachelor's degree preferred.
• Case management experience required.
• Excellent engagement, communication, and organizational skills.
• Ability to work independently and responsibly in community settings.
• Valid driver's license and acceptable driving record.
• Personal vehicle with current, valid vehicle registration and inspection.
• Proof of automobile insurance with the following minimum coverage levels: $100,000 bodily injury per person; $300,000 aggregate bodily injury; $100,000 property damage, or $300,000 combined single limit.
• Ability to complete required Residential Counselor and behavioral-support training, including physical intervention/restraint training when necessary.

Responsibilities

• Coordinate services, appointments, referrals, meetings, and other supports.
• Complete required assessments and case management documentation.
• Play an active role in treatment planning, transition and discharge planning, and aftercare.
• Support youth in developing independent-living skills and accessing community resources.
• Serve as a link between youth, families, DCF, schools, employers, community providers, and the treatment team.
