Job summary

The Home for Little Wanderers in Boston seeks an Outreach/Case Manager for the PACT‑Y program. You will provide intensive, community‑based support to youths with behavioral health needs as part of a multidisciplinary team.

You will engage youth and families, coordinate care across systems, and help implement individualized treatment goals while maintaining strong relationships and accurate documentation.

Qualifications

• High School diploma or GED; Bachelor's degree preferred.
• Must work with people of different backgrounds, languages and ethnicities.
• Driver’s license required.

Responsibilities

• Deliver community-based outreach and case management services in home, school, and other natural settings.
• Engage youth and caregivers to build consistent, trusting relationships.
• Support implementation of individualized treatment plans in partnership with the team.
• Assist with crisis prevention and intervention, utilizing a team-based approach.
• Coordinate care with providers, including mental health, medical, educational, and state agencies.
• Support development of life skills, emotional regulation, and social functioning.
• Connect youth and families to community resources and supports.
• Participate in daily team meetings and ongoing treatment planning.
• Complete timely and accurate documentation in compliance with program standards.
• Provide transportation support for youth as needed.
