Job summary

Breaking Ground is seeking a Case Manager, Transitional Housing, to provide case management for people experiencing homelessness at The Andrews on the Bowery, a 146-unit Safe Haven. You will assist with housing applications, connect clients to psychiatric and medical services, and support move to permanent housing.

The role requires a Bachelor’s Degree or equivalent, 2+ years of relevant experience, and strong writing/communication skills. On-site, 35-hour week with on-site services included.

Qualifications

• Bachelor’s Degree or equivalent work experience.
• Two years of experience working with homeless adults and substance abuse issues preferred.
• Experience with harm reduction and motivational interviewing techniques strongly preferred.
• Excellent writing and communication skills.
• Valid driver’s license in NY, NJ, CT, or PA preferred.

Responsibilities

• Provide direct services to a caseload of approximately 15 clients, including assessments, benefits assistance, and services to support housing and recovery goals.
• Conduct psychosocial evaluations and connect clients to psychiatric and medical services.
• Assist clients in obtaining identification, public benefits, and other income sources; maintain documentation.
