What to Expect (Job Responsibilities):
- Coordinate follow-up care plan needs for members by scheduling appointments or enrolling members in programs
- Assess member compliance with medical treatment plans via telephone or through on-site visits
- Identify barriers to plan compliance and coordinate resolutions
- Recommend treatment plan modifications and determine the need for additional services
- Prepare reports to document case and compliance updates
What is Required (Qualifications):
- High School diploma or equivalent and a minimum of 1 year related experience
- Strong oral, written, and interpersonal communication skills
- Problem-solving and analytical skills
- Ability to work Eastern Standard Time or Central Standard Time
- Experience with case management or care coordination preferred
How to Stand Out (Preferred Qualifications):
- Certification as a nurse assistant or medical assistant
- Bilingual candidates are preferred
- Experience in healthcare-related service roles
- Familiarity with local, state, or federally funded programs
- Strong facilitation skills
