Position Details

Title: Outreach Care Specialist - Transitions of Care (Virtual)

Location: Fully remote, with required in-person training sessions. Flexibility and autonomy are supported.

Schedule: Must be able to work Eastern or Central Standard Time.

Role Overview

The Outreach Care Specialist ensures member treatment plans are followed for less complex cases and proactively seeks ways to improve health outcomes and meet quality targets.

Key Responsibilities

• Coordinate follow-up care and enroll members in programs.
• Assess compliance via calls or visits.
• Identify and resolve barriers to care.
• Suggest process improvements to meet quality goals.
• Recommend treatment modifications and additional services.
• Refer members to local, state, or federally funded programs.
• Coach members on reducing health risks.
• Document case updates and compliance reports.
• Build and maintain relationships with relevant agencies.

Minimum Requirements

• High school diploma or equivalent, with at least 1 year of related experience or equivalent background.

Preferred Skills

• Certified nurse assistant or medical assistant preferred.
• Bilingual candidates preferred.
• Strong communication, problem-solving, facilitation, and analytical skills, especially for URAC accredited areas.
• Bilingual/multilingual skills may be required for certain units.
