What to Expect (Job Responsibilities):
- Coordinate follow-up care plans for members by scheduling appointments and enrolling them in programs
- Assess member compliance with medical treatment plans via phone and on-site visits
- Identify barriers to compliance and coordinate resolutions
- Recommend modifications to treatment plans and determine the need for additional services
- Establish and maintain relationships with local, state, or federally funded programs
What is Required (Qualifications):
- High School Diploma or equivalent
- Minimum of 1 year related experience in a healthcare or similar setting
- Strong oral, written, and interpersonal communication skills
- Problem-solving and analytical skills
- Ability to work Eastern Standard Time or Central Standard Time
How to Stand Out (Preferred Qualifications):
- Certification as a nurse assistant or medical assistant
- Bilingual candidates are preferred
- Experience in case coordination or care management
- Familiarity with community resources and healthcare programs
