Job summary

Transition Projects, Inc. is seeking a Supportive Housing Case Manager at the Macadam Office in Portland to provide outreach-based services helping individuals stabilize and thrive in Permanent Supportive Housing.

You will work with participants to assess needs across medical, mental health, substance use, income development, education, and social supports, coordinating with landlords and community providers.

Qualifications

• Bachelor’s degree in a related field or, alternatively, 2 years of experience in social services.
• Ability to interact with multiple agencies and advocate for participants' needs.
• Knowledge of chemical dependency and mental health issues and resources.
• Familiarity with community resources for the homeless and housing insecure.

Responsibilities

• Provide outreach-based support to PSH participants to promote housing stability.
• Assess service needs across medical, mental health, substance use, income development, and basic needs.
• Coordinate with landlords, property owners, and community providers to deliver services.
• Maintain participant files and HMIS documentation; prepare monthly case management reports.
