Job summary

Helping Hands Reentry Outreach Centers in Lincoln City, Oregon, is seeking a Case Manager to support participants in navigating resources and pursuing stable housing. You’ll build supportive relationships, develop Individual Reentry Plans, and help people access housing, healthcare, and community services.

Qualifications

• High school diploma or equivalent.
• At least one year of case management or social services experience.
• Strong interpersonal, verbal, and written communication skills.
• Careful documentation and basic use of technology, including email, Word and Excel, and data entry.
• Trauma-informed, person-centered approach and ability to work with participants and partners.

Responsibilities

• Build rapport with participants and learn their needs through conversations, records review, and consultation.
• Meet with assigned participants monthly to develop and update Individual Reentry Plans (IRPs), set goals, review progress, and identify next steps.
• Support participants in understanding and meeting program requirements.
• Help participants access housing, healthcare, SNAP, DMV appointments, employment supports, and other services.
• Coordinate with professionals and community partners involved in participants’ success.
• Respond to crises using trauma-informed de-escalation and connect participants with timely resources.
• Develop and maintain a network of community resources, including medical and mental health services and supports.
• Build relationships with landlords, housing authorities, and affordable-housing partners; stay informed about housing opportunities.
• Provide education and resources to help participants secure and maintain permanent housing, including rental assistance.
• Maintain accurate documentation and protect participant confidentiality.
