Job summary

Healogics is the nation’s leading provider of advanced wound care, delivering care across 600+ sites. The Clinical Patient Navigator role blends administrative and clinical duties to support an outpatient wound care center and streamline patient flow.

In this position you will handle scheduling, patient intake, and chart maintenance while assisting clinicians with wound care procedures as needed. Strong communication and organization are essential to succeed in a fast-paced environment.

Qualifications

• High School Diploma or GED required; an associate degree is preferred.
• Certification as CMA, EMT or CNA from a recognized program is required/licensed in-state.
• Minimum of 2 years of office administration experience, preferably in healthcare.

Responsibilities

• Coordinate with Center leadership to maximize daily patient census.
• Greet patients and visitors; answer and route calls; manage scheduling, rescheduling and reminders.
• Assist RN/Provider with wound-related patient care per protocols and competencies.
• Gathers patient information and prepares charts; manages charts and data entry.
• Coordinate ancillary testing; arrange patient transportation; maintain supplies.
