Responsibilities

• Conduct psychosocial assessments to document patient history and identify treatment focus.
• Participate in development and implementation of treatment plans and ensure care plans are carried out.
• Provide individual, group and family therapies and conduct didactic groups using professional modalities.
• Document treatment provided in medical records in a timely manner.
• Attend regular treatment team meetings and coordinate with the interdisciplinary team.
• Develop and coordinate individualized aftercare plans for discharge needs.
