• Licensure: LPC, LSW or LCSW required.
• Education: MSW or master's degree in a recognized mental health field required.
• Experience: Two years of partial hospital experience

Responsibilities

• Conduct psychosocial assessments to document patient history and identify preliminary issues for treatment focus by interviewing the patient and their family members.
• Participate in development and implementation of treatment plans, ensuring concise, realistic care plans are carried out.
• Provide individual, group and family therapies and conduct didactic groups using various professional treatment modalities.
• Document treatment provided to patient in medical record legibly and timely.
• Attend regular treatment team meetings to provide social service perspective to case management and coordinate with the treatment team.
• Develop and coordinate individualized aftercare plans for discharge needs with input from family and community resources
