Job summary

Lancaster Behavioral Health Hospital is seeking an Outpatient Therapist for a part-time evening role. The position offers hybrid work once fully trained, with four days per week and five hours per day, combining on-site time with remote work.

Responsibilities include psychosocial assessments, treatment planning, individual/group/family therapies, and meticulous documentation. Licensure in LPC, LSW, or LCSW and an MSW or related master’s degree are required; two years of partial hospital

Qualifications

• Education: MSW or master's degree in a recognized mental health field required.
• Licensure: LPC, LSW or LCSW required.
• Two years of partial hospital experience preferred.

Responsibilities

• Conduct psychosocial assessments to document patient history and identify treatment focus.
• Participate in development and implementation of treatment plans and care targets.
• Provide individual, group and family therapies using various modalities.
• Document treatment provided in the medical record in a timely, legible manner.
• Attend regular treatment team meetings to coordinate case management with the interdisciplinary team.
• Develop and coordinate individualized aftercare plans using team, family, and community resources.
