Job summary

Beth Israel Lahey Health is seeking a Team Nurse RN to provide direct patient care in outpatient primary care settings. You will supervise and educate Medical Assistants, coordinate care with providers, and support preventive and chronic disease management under a standardized protocol.

You will administer vaccines and medications, manage INR per protocol, and ensure accurate documentation in the EHR while delivering high-quality, patient-centered care.

Qualifications

• Graduate from an accredited Nursing Program (Bachelor's preferred).
• Massachusetts RN license is required; Basic Life Support (BLS) certification required.
• 1–3 years of related work experience preferred; new graduates considered.
• Strong interpersonal and patient education skills; able to communicate clearly with patients.
• Proficient with Microsoft applications (Outlook/Word/Excel/PowerPoint/Access) and data management.

Responsibilities

• Coordinate and facilitate quality clinical management across an outpatient population.
• Collaborate with providers and care teams to support preventive care and chronic disease management.
• Provide skilled telephonic triage, patient education, and care coordination.
• Manage INR and patient education for Coumadin per protocol.
• Administer vaccines and medications per orders and policies.
• Perform wound care, point-of-care testing, and documentation in EHR.
• Follow up on abnormal test results and coordinate referrals.
