Job summary

Qualifications

• Graduate of an accredited Nursing Program.
• Massachusetts RN license required.
• 1-3 years related work experience preferred or new graduates considered.

Responsibilities

• Coordinate and facilitate quality clinical management across the outpatient population.
• Collaborate with providers to identify needs for preventive care and care coordination.
• Provide nursing visits including AWVs and CCM for BIDHC patients.
• INR management and teaching for Coumadin per protocol.
• Administer vaccines and medications per provider orders and policy.
• Document all patient care in EHR and coordinate tests/referrals.
• Monitor abnormal test results and ensure appropriate follow-up.
