Job summary

Beth Israel Lahey Health is seeking a Team Nurse to deliver RN/LPN level care in an outpatient primary care or family medicine setting. You will educate and supervise Medical Assistant staff, coordinate care transitions, and support preventive and chronic care management in a team-based environment.

Responsibilities include triage, patient education, vaccine administration, and accurate documentation in the EHR.

Qualifications

• Graduate from an accredited Nursing Program.
• RN or LPN license is required.
• CPR certification.
• 1–3 years related work experience.
• Strong interpersonal and patient education skills.

Responsibilities

• Coordinate and facilitate quality clinical management for a broad outpatient population.
• Collaborate with providers to identify preventive care and chronic disease needs.
• Provide skilled telephonic triage and patient education.
• Manage INR and education for Coumadin patients.
• Administer vaccines and medications per orders and policy guidelines.
• Document patient care in the EHR and schedule tests/referrals.
• Oversee inventory of supplies and provide training to MA staff.
