Qualifications

• Graduate with a bachelor’s degree in nursing from an accredited school.
• Current RN licensure in good standing and CO licensure by date of hire.
• Substitution: education/experience may substitute for degree on a year-for-year basis.

Responsibilities

• Care Coordination across service lines for mutual patients.
• Care coordination navigation and referral processing for complex needs.
• Health coaching, including blood pressure and diabetes visits, nutrition counseling.
• Documentation of patient care in the EMR and adherence to quality measures.
• Support and training of medical assistants and student staff.
