University of Colorado College of Nursing seeks a Primary Care RN Case Manager for a full-time, 1.0 FTE position based at Sheridan Health Services in Denver, CO. The role coordinates patient care across clinic services, triages calls, and helps families access resources.

The RN provides clinical care for children and adults, supports case management, and collaborates with the care team to ensure high-quality, cost-effective services in an outpatient setting.

Qualifications

• Bachelor’s degree in nursing from an accredited school of nursing.
• Current Colorado RN licensure in good standing by date of hire.
• Must obtain BLS/CPR certification by date of hire.

Responsibilities

• Coordinate patient care across clinic services and connect patients with specialty care.
• Triage incoming patient calls and provide health coaching and education.
• Document patient care in the EMR and participate in quality improvement activities.
• Support onboarding of students and staff; supervise Medical Assistants if applicable.
