Job summary

Qualifications

• Minimum: Associate's Degree in Nursing.

• Current RN license in Ohio; BLS preferred.
• Minimum 2 years nursing experience in ambulatory/outpatient setting.

Responsibilities

• Engage in population-appropriate communication and patient teaching.
• Assess patients and develop care plans during outpatient visits.
• Coordinate with interdisciplinary team to optimize patient care.
• Document nursing assessments and order referrals as needed.
• Support transitions of care and follow-up scheduling.
