Job summary

Silver Creek Physical Therapy, a member of Therapy Partners Group, is seeking a dedicated Outpatient Physical Therapist Assistant to join our team in the San Jose area. The role involves assisting with patient treatment plans, documenting progress, and educating patients under the supervision of a licensed PT.

Requirements include an accredited Physical Therapy Assistant degree and current state licensure or eligibility.

Qualifications

• Associate Degree from an accredited institution.
• Current state licensure or eligibility for licensure as a Physical Therapist Assistant.
• Outpatient experience preferred but not required.
• Ability to work collaboratively in a team-oriented environment.
• Commitment to providing exceptional patient care.

Responsibilities

• Patient Follow Up: Conduct therapeutic exercises and modalities as instructed by the physical therapist.
• Treatment Planning: Assist physical therapists in developing and implementing patient treatment plans.
• Therapeutic Interventions: Implement manual therapy, exercise prescriptions, modalities, and patient education.
• Documentation: Use Raintree, monitor and document patient progress during treatment sessions, reporting changes to the therapist.
• Patient Education: Educate patients and families on exercises, pain management techniques, and care plans.
• Professional Atmosphere: Collaborate with the healthcare team to ensure optimal patient care and safety.
