Silver Creek Physical Therapy, a premier outpatient orthopedic provider in San Jose, seeks a dedicated Physical Therapist Assistant to join the team.

You will support therapists with exercise programs, manual therapy, and patient education, while using Raintree for documentation. Outpatient setting experience is preferred, and licensure or eligibility is required.

Qualifications

• Associate Degree in Physical Therapy Assistant from an accredited program.
• Current state licensure or eligibility for licensure as a Physical Therapist Assistant.
• Outpatient experience is preferred but not required.
• Ability to work collaboratively in a team-oriented environment.
• Commitment to providing exceptional patient care.

Responsibilities

• Conduct therapeutic exercises and modalities as instructed by the physical therapist.
• Assist physical therapists in developing and implementing patient treatment plans.
• Implement various therapeutic techniques, including manual therapy and exercise prescriptions.
• Monitor and document patient progress using Raintree and report changes to the physical therapist.
• Educate patients and families on exercises, pain management, and care plans.
• Collaborate with the healthcare team to ensure optimal patient care and safety.

Silver Creek Physical Therapy, a member of Therapy Partners Group, has been the premier provider for outpatient orthopedics serving San Jose and surrounding communities for over 20 years.

We are seeking a dedicated and skilled Outpatient Physical Therapist Assistant to join our dynamic team.

Key Responsibilities:

• Patient FollowUp: Conduct therapeutic exercises and modalities as instructed by the physical therapist.
• Treatment Planning: Assist physical therapists in developing and implementing patient treatment plans.
• Therapeutic Interventions: Implement various therapeutic techniques, including manual therapy, exercise prescriptions, modalities, and patient education.
• Documentation: While using Raintree, Monitor and document patient progress during treatment sessions, reporting changes to the physical therapist.
• Patient Education: Educate patients and families on exercises, pain management techniques, and care plans
• Professional Atmosphere: Work collaboratively with the healthcare team to ensure optimal patient care and safety.

Pay: $40-44/hour depending on experience

Qualifications:

• Associate Degree as Physical Therapy Assistant from accredited institution.
