Silver Creek Physical Therapy, a member of Therapy Partners Group, has been the premier provider for outpatient orthopedics serving San Jose and surrounding communities for over 20 years. We are seeking a dedicated and skilled Outpatient Physical Therapist Assistant to join our dynamic team.

Responsibilities include patient follow up, treatment planning, therapeutic interventions, documentation using Raintree, patient education, and collaboration with the healthcare team.

Qualifications

• Associate degree required from an accredited program.
• Current state licensure or eligibility for licensure as PTA.
• Outpatient experience preferred but not required.
• Ability to work collaboratively in a team-oriented environment.
• Commitment to providing exceptional patient care.

Responsibilities

• Patient follow-up: perform therapeutic exercises and modalities as directed by the PT.
• Treatment planning: assist PTs in developing and implementing patient plans.
• Therapeutic interventions: manual therapy, exercise prescriptions, modalities, and patient education.
• Documentation: document patient progress using Raintree and report changes to the PT.
• Patient education: educate patients and families on exercises, pain management, and care plans.
• Maintain a professional atmosphere and collaborate with the healthcare team.

Key Responsibilities:

• Patient Follow Up: Conduct therapeutic exercises and modalities as instructed by the physical therapist.
• Treatment Planning:Assist physical therapists in developing and implementing patient treatment plans.
• Therapeutic Interventions: Implement various therapeutic techniques, including manual therapy, exercise prescriptions, modalities, and patient education.
• Documentation: While using Raintree, Monitor and document patient progress during treatment sessions, reporting changes to the physical therapist.
• Patient Education:Educate patients and families on exercises, pain management techniques, and care plans
• Professional Atmosphere: Work collaboratively with the healthcare team to ensure optimal patient care and safety.

Pay: $40-44/hour depending on experience
