Advocate Health seeks a licensed Doctor of Physical Therapy to join an experienced outpatient rehabilitation team in Wisconsin. You will assess, plan and implement evidence-based PT care, with opportunities for mentorship and continuing education.

New grads welcome; strong patient outcomes and collaboration with referring providers are emphasized. Eligible for a $20,000 sign-on bonus; 40-hour work week with variable schedule at a predominantly outpatient site.

Qualifications

• Doctorate in Physical Therapy required.
• Licensed Physical Therapist in the state where you work.
• Strong communication, organization and problem-solving skills.

Responsabilités

• Interprets referrals and performs musculoskeletal, neuromuscular, cardiopulmonary and integumentary evaluations.
• Develops and implements individualized treatment plans with measurable goals.
• Monitors outcomes and adjusts plans; communicates progress to physicians and team.
• Completes discharge planning including home programs and community referrals.
• Documents all care and supports billing and scheduling per policy.
• Maintains safety and infection control standards and ensures equipment readiness.
• May supervise PT assistants and therapy extenders following applicable regulations.
