Job summary

Qualifications

• Bachelor’s degree in Physical Therapy required.
• Wisconsin Physical Therapist license required.
• Basic Life Support (BLS) for Healthcare Providers certification within 6 months if not already held.

Responsibilities

• Interprets physician referrals and completes musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations.
• Establishes a plan of care and implements individualized treatment based on evaluation results and functional goals.
• Monitors outcomes and adjusts treatment plans; communicates progress to physicians.
• Completes discharge planning including home programs and referrals for community services.
• Documents patient care per department policy, including progress notes and discharge summaries.
• Maintains clean and orderly treatment area; supports equipment handling and patient positioning.
• Demonstrates modalities usage and knowledge of indications/contraindications.
• Participates in performance improvement activities and manages assigned caseload.
