Job summary

Advocate Health in Sheboygan, WI, is seeking a full-time Physical Therapist for the outpatient rehabilitation clinic. Monday–Friday, first shift, 40 hours per week, with a $5,000 sign-on bonus. Pay range: $41.10–$61.65 per hour.

You will interpret referrals, perform musculoskeletal, neuromuscular, and cardiopulmonary evaluations, develop and adjust care plans, discharge patients with home programs, document thoroughly, and support team goals. Requires Wisconsin PT license and BLS within 6 months.

Qualifications

• Bachelor’s degree in Physical Therapy is required.
• Wisconsin Physical Therapist license required.
• BLS certification within 6 months.

Responsibilities

• Interpret referrals and perform musculoskeletal, neuromuscular, and cardiopulmonary evaluations.
• Develop and implement plan of care with goals.
• Monitor outcomes and adjust treatment plans; provide updates to physicians.
