Job summary

Advocate Health seeks a Physical Therapist for outpatient rehabilitation in Wisconsin. You will evaluate patients, develop and implement individualized care plans, and monitor progress to achieve functional goals. Role requires active licensure and documentation per department policies.

Responsibilities include patient discharge planning, home exercise programs, and collaboration with physicians. You may participate in weekend/holiday rotations and adhere to hospital based certifications.

Qualifications

• Bachelor's Degree in Physical Therapy required.
• Wisconsin Physical Therapy license required or eligible.
• BLS for Healthcare Providers certification (AHA) within 6 months if required.

Responsibilities

• Interprets physician referrals and conducts musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations.
• Establishes plan of care and implements treatment based on evaluation results and goals.
• Monitors outcomes and adjusts treatment plans; communicates with physicians.
