Illinois Bone and Joint Institute, LLC (IBJI) seeks a Physical Therapist to provide outpatient rehabilitation services in a collaborative care model with our physicians to optimize patient outcomes.

Qualifications

• Graduate from accredited Physical Therapy Program.
• Physical Therapist New Grads eligible to apply.
• Licensed or eligible to sit for boards in the state of Illinois.
• Ability to work in a fast paced environment.
• Professional and effective writing and verbal communication skills.
• Ability to lift greater than 25 pounds.
• Demonstrates good understanding of proper body mechanics for lifting, pulling, pushing.
• Ability to stand and sit for sequences of time greater than 30 minutes.
• Ability to work on a computer for lengthy times during the day.

Responsibilities

• Exceptional patient care.
• Interprets physician orders, performs initial evaluations and establishes treatment plans and functional goals.
• Responsible for upgrading and amending the treatment goals as necessary during the episode of care.
• Written documentation on patient status to physicians per compliance guidelines of IBJI rehab.
• Determine appropriate discharge timeline and appropriate follow up.
• Supervision of Rehabilitation Technicians with patient care and administrative staff as needed.
• Compliance with daily paperwork and accurate documentation of all treatment in patient charts.
• Maintains active state license by earning CEUs and IBJI sponsored opportunities.
• Develops positive relationships with physician teams and participates in the patient care team.
