Qualifications

• Graduate of an accredited Physical Therapy program with license or eligibility in Illinois.
• Strong written and verbal communication skills and professional writing ability.
• Ability to work in a fast-paced outpatient setting and collaborate with physicians.

Responsibilities

• Provide exceptional patient care in outpatient rehabilitation.
• Interpret physician orders, perform initial evaluations and establish treatment plans and goals.
• Adjust treatment goals as needed during the episode of care.
• Document patient status in charts per compliance guidelines.
• Determine discharge timelines and follow-up plans.
• Supervise rehabilitation technicians and coordinate with clinical staff.
• Adhere to billing policies and ethical billing practices.
• Maintain active state license through CEUs and IBJI educational opportunities.
• Develop relationships with physician teams and participate in care planning.
