Job summary

Advocate Health is seeking a licensed Physical Therapist to evaluate and treat patients across musculoskeletal, neuromuscular, cardiopulmonary, and integumentary systems. You will establish plans of care, implement treatment, monitor outcomes, and adjust based on patient response while communicating with prescribers and the care team.

You will discharge patients with home programs, provide education to patients and families, and document care per policy.

Qualifications

• Doctorate degree in Physical Therapy from an accredited school.
• Maintains continuing education requirements as mandated by state standards.
• Licensed Physical Therapist in the practicing state.

Responsibilities

• Evaluate patients and develop plans of care for musculoskeletal, neuromuscular, cardiopulmonary, or integumentary systems.
• Implement treatment and monitor outcomes with adjustments as needed.
• Provide discharge planning and home programs.
• Document evaluations, progress notes, and discharge summaries per policy.
• Coordinate with physicians and team; participate in improvement and safety initiatives.
