Job summary

Advocate Health seeks a licensed Physical Therapist to evaluate, treat, and discharge patients within its care continuum. You will develop individualized plans of care, select therapeutic modalities, and monitor progress toward functional goals.

Qualifications

• Doctorate degree in Physical Therapy from an accredited school or as required by graduation date.
• Licensed Physical Therapist in the state of practice.
• Basic Life Support (BLS) for Healthcare Providers (AHA).

Responsibilities

• Interpret physician referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations.
• Establish a plan of care and implement treatment based on evaluation results and functional goals.
• Monitor outcomes and adjust the treatment plan based on patient response.
• Plan for and complete patient discharge including home programs and family teaching.
• Document evaluations, progress notes, and discharge summaries per policy; ensure proper billing and productivity requirements.
• Maintain patient safety and participate in quality improvement activities.
