Job summary

Advocate Health seeks a Physical Therapist to evaluate, plan and direct PT services in an inpatient acute setting. This role works with physicians to restore function and maximize patient independence, delivering therapy seven days a week, including holidays.

Responsibilities include evaluating patients, designing care plans with modalities and education, documenting progress, coordinating discharge, and supervising PT staff per licensure.

Qualifications

• Bachelor's degree in Physical Therapy required.
• North Carolina Physical Therapy license required.
• Basic Life Support (BLS) certification required.

Responsibilities

• Provides direct therapy services to patients admitted to a level of care where service provision is required 7 days a week, including holidays.
• Evaluates patients and sets up treatment plans including modalities, exercise programs, and patient education.
• Gathers and interprets data from the medical record, physician reports, patient and family to document care.
• Documents evaluations, tests, progress notes, discharge notes, and related charges.
• Communicates evaluation and progress to physicians and care team; coordinates schedules and discharge planning.
• Provides supervision of PTA's and Rehab Tech/Aide as required for licensure.
