Qualifications

• Strong communication skills and ability to work in a complex health system.

Responsibilities

• Provide individual therapy aligned with patient diagnosis and needs.
• Offer crisis intervention to new and current patients as needed.
• Develop and monitor individualized treatment plans and interventions.
• Provide case management and utilization review services for assigned caseload.
• Make appropriate referrals and liaise with internal/external services for continuity of care.
• Engage in supervision, peer reviews, and care-management initiatives.
• Contribute to community education, outreach, and marketing to develop new referrals.
• Complete documentation on treatment plans, discharge summaries, and related records.
