Ascension ProCare Center in Melrose Park, IL, is seeking an experienced behavioral health clinician for full-time outpatient work. You will perform high-level diagnostic assessments and develop tailored treatment plans, delivering individual, group, and family therapy within a multidisciplinary team.

The role requires a master’s degree and Illinois licensure credentials (LCPC/LCSW) with a focus on patient-centered care.

Qualifications

• Licensure/certification required by Illinois Department of Financial and Professional Regulation.
• Master's degree required.
• LCPC/LCSW licensure preferred or required.

Responsibilities

• Clinical Assessment & Diagnosis: perform high-level diagnostic assessments within psychosocial context.
• Treatment & Psychotherapy: develop individualized treatment plans with measurable goals and provide evidence-based therapy.
• Multidisciplinary Collaboration: collaborate with a care team and lead or participate in care conferences.
• Schedule: Monday–Wednesday 11:00 am–8:00 pm, Thursday 9:00 am–6:00 pm, Friday 9:00 am–3:30 pm.
